The Raptors trailed by nine early in the fourth but used a 12-2 run to take the lead for good. Norman Powell scored eight of his 16 points in the quarter and Siakam had seven as Toronto won for the eighth time in 11 games.
Jimmy Butler scored 21 points and Bam Adebayo had 18 points and eight rebounds for the Heat, who have lost three of four. Goran Dragic scored 14 points but shot just 4 for 12.
Toronto was without OG Anunoby (calf) and Malachi Flynn (health and safety protocols). The Raptors are already without leading scorer Kyle Lowry (foot), who is expected to miss at least another week.
The Raptors beat the Miami Heat 98-90 on Monday. With 24 points from Fred VanVleet and 23 points from Pascal Siakam, the Raptors rallied in the fourth quarter to take the lead for good. The Heat were without Tyler Herro and Vic Oladipo.
The Raptors are already without leading scorer Kyle Lowry. The Raptors host Oklahoma City on Thursday night while the Heat host Golden State on Wednesday night.
